Job Description

We are seeking two Clinical Advisors/Acute trust liaison Clinicians. One 37.5hrs and one 30hrs to work within Royal United Hospital Bath/Access to Care, Chippenham, and Salisbury Foundation Trust Salisbury/Access to care Chippenham. The role is to provide advice on treatment/care pathways face to face in the acute hospital setting and by telephone with health and social care professionals in accordance with agreed care pathways.

You will hold key relationships internally with all members of Access to Care and urgent care services. Externally with nursing, therapy, medical and other allied professionals related to the Community Teams in addition to wider health community service users, patients and carers, acute hospital and community trusts, General Practitioners, social care staff, care homes and ambulance crews. As an Acute trust Liaison Clinician you will be working closely with Wiltshire Health and Care supporting with discharges from the acute hospitals into a variety of community services

Working within the multidisciplinary team at Medvivo, the key responsibilities for this role are:

  • To provide clinical advice and support to health and social care professionals in accessing the most appropriate care pathway for their patients
  • To provide advice to patients and carers where appropriate who access care by telephone
  • To ensure the patient remains the central focus of all decision making
  • To complete face to face liaison with patients and families re discharge pathways in the acute setting
  • To lead the clinical decision making within Access to Care taking into account capacity and constraints within the community
  • To be responsible for the telephone assessment of patients referred to the service acting as an expert in planning quality care at the highest level
  • To ensure own clinical credibility by demonstrating accountability and maintaining clinical skills
  • To assist in screening and prioritising referrals to the Community teams, including Urgent Care @ Home, Community hospitals, Discharge to access and Intensive rehab beds
  • Assist with Conference calls, Call reviews and Mentoring to support the Clinical leads
  • To be able to recognise the limits of your own clinical knowledge and competence and seek further advice and assistance where appropriate
  • To maintain a good knowledge of infection control including local procedures for Wiltshire and surrounding District and Community hospitals
  • To obtain consent for any referrals made
  • To instigate escalation procedures where appropriate and where necessary
  • To adhere to the Medvivo Lone Working Policy if working alone in the Acute Hospitals

You may be a Nurse/ Therapist/ Paramedic/Assistant Practitioner or Therapy Assistant with evidence of ongoing continuous professional development. You should have a minimum of 5 years’ experience within a broad range of care and evidence of the ability to maintain effective working relationships with peers, colleagues, patients and their families.

From your experience, you’ll have excellent judgement and decision making skills, the ability to remain calm and professional under pressure and be familiar in the use of IT systems.

This role is made up of various shift patterns from 0800-2100 weekdays and 0800-1600 at weekends

Hourly rate : Competitive and dependent upon experience
