We are seeking Clinical Pharmacists to join our team of remote working clinicians on a fully remote and self-employed basis.
You
Will be an experienced clinical pharmacist who MUST have general practice experience.
Working as part of a multi-disciplinary practice team taking responsibility for areas of chronic disease management and undertake clinical medication reviews, with an ability to apply effective clinical judgement. To contribute to improve health outcomes in long term conditions within the practice team. To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment. Reviewing patient’s acute medicines requests, completing discharge and clinic letters, providing structured medication reviews and advice and support for patients with self-limiting conditions.
Requirements
- Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
- Independent prescribing qualification
- Minimum of 2 years’ experience as a pharmacist, demonstrated within a practice portfolio.
- To have qualified from an accredited training pathway that equips the post holder to be able to practice and prescribe safely and effectively in a primary care setting and in order to deliver the key responsibilities of the role.
